Introduction: Chronic renal disease (CRD) is a progressive and irreversible medical condition in which the kidneys gradually lose their ability to function properly, and represents a clear public health problem. In 2019, in the Region of the Americas, kidney disease was responsible for 254,028 deaths. CRD develops slowly, often without obvious symptoms in its early stages, and this causes a serious problem, since the symptoms usually appears when the disease is already in an advanced stage. Objective: To promote self-care at home for patients with kidney disease and peritoneal dialysis replacement therapy. Method: A care plan was elaborated based on NANDA, NIC and NOC taxonomy, in addition to the implementation of the MNA (Mini Nutritional Assessment) scale. Conclusions: CRD is a progressive condition that affects especially older adults with diseases such as diabetes and hypertension. Gradual loss of kidney function impacts waste elimination and fluid balance, affecting the patient’s quality of life and generating high health care costs. Nursing care at the patient’s home is crucial to identify needs, promote self-care, improve health conditions and thereby reduce complications; thus contributing to an integral management of the individual with the participation of the family.
