Introduction: Technovigilance and safety in drug administration play a crucial role in the context of adverse events in the operating room. It involves the surveillance, monitoring and systematic reporting of technological systems, medical devices, as well as updating processes, evaluation through indicators and essential actions to guarantee patient safety and quality of care. Objective: Identify the frequency of adverse events in an operating room area that allows classifying their causes and establishing monitoring strategies to reduce risks. Method: Retrospective, quantitative and descriptive study of adverse events in the operating room between January 2021 to October 2023 in a healthcare center in Mexico, with non-probabilistic convenience sampling. Results: 513 adverse events were recorded, of which 44 (0.5%) occurred in the operating room. The frequency of adverse events increased from 2 in 2021 to 22 in 2022 and 20 in 2023. It is important to note that the care and activities of the nursing staff were related to the frequency of adverse events, although most reports were reported in a timely manner by the same staff. Conclusions: The adverse events at the center under study were highly preventable, so the implementation of technovigilance and the strengthening of PHEAs through updating and evaluation will bring benefits that impact patient safety, where nursing plays a major role.
